Program Overview & Services Provided
With the help of our qualified staff and volunteer staff, Hope Ministries is able to provide a dynamic and effective program that includes the following:
- Project Hope: Case Management, Resource and Referrals
- Hope for a Brighter Future: Occupational preparedness workshops.
- Hope for Mental Health: Group and individual Counseling super-vised by an on-site Marriage and Family Therapist
- Children of Hope: Activity centered group and individual counseling for our children who are expe-riencing the crisis of being homeless.
- Interns of Hope
Screening
A first-level decision about whether the client is eligible for our program and/or would have a priority for those services. Screening determines who receives assistance and where they will be placed (Raymus House or Hope Family Shelter.)
Screening is the general process between the potential client’s first point of contact and the decision to admit or not admit the client to a program. The screening process can include a referral in the case that the client does not qualify for our program. Screening may or may not result in program admission.
Intake
First Level intake will take place with the director of the organization and/or director of client services.
Intake can include but is not limited to:
- Information – specific facts about a program, such as its location, services provided, eligibility requirements, hours of operation, and contact information.
- Referral – referring a client to a particular program for possible help, without any obligation on the part of the receiving program to actually offer or provide the help.
- Verification – the gathering and review of information to substantiate the crisis situation and support program eligibility (drug screening) and priority determinations.
- Admission – using authority to admit the client into the program.
Assessment
This assessment will consist of a mental health evaluation and deeper program explanation that will evaluate client needs and create a client centered individualized treatment plan unique to each individual client to effectively working the client successfully through program.
Additional elements of this assessment are as follows:
- Case Management Assignment – with the clinical director’s oversight, a case manager will be assigned to oversee the client’s weekly activities and program involvement. The case manager will work with both the program director and clinical director to ensure all client needs are being assessed and met so that the goal of occupational and residential independence is being attained.
- Addiction Severity Index (ASI) Assessment – a deeper level of inquiry into the actual problem and the client’s strengths and needs related to solving the problem. Assessment helps with service-matching and provides the information needed to determine the expected type, intensity, and duration of assistance a client or household might receive.
- Genogram – a family history outline that is done by the marriage and family therapist.
- Service-Matching – evaluation of client’s resources that they may not realize they have that can be utilized in their progress through the program. Effective applicable referrals to local agencies that will further clients efforts in progress through their time in program.
Random drug testing is conducted at the discretion of the director of client services in order to maintain eligibility in the program.
Group counseling is offered to all residents. Meetings are as follows:
